****    New Leader Orientation    ****

#2

Wednesday, October 28, 2009

6:30 – 9:00 PM

UCCE Meeting Room

501 Texas St., Fairfield

All new leaders and returning leaders who have had a break in service of a year or more must have orientation training, livescan fingerprinting; and be covered by the University Liability Insurance Program before they are permitted to function as a volunteer leader in 4-H.  We will cover youth protection safety guidelines, leader responsibility, safety, publications, utilizing Jr. & teen leaders and answer any questions you may have.

We will have a Livescan technician present to take fingerprints.  Call or email Valerie Williams to register at vawilliams@ucdavis.edu or (707) 784-1319.  You must bring a photo ID and payment ($15.00) in the form of cash, check, VISA or MasterCard in order to complete fingerprinting.  Complete the Request for Livescan Service form (link below) with your personal information (in the 3rd block of the form, beginning with “Name of Applicant” ending with your “Social Security Number”).  The form is in triplicate, you will only need to type your information on page one, pages 3 & 4 will self-populate with your information.  Print out all three pages of the completed forms and bring with you to the Solano County 4-H Leaders Orientation on Wednesday, October 28th

Until leaders have attended orientation and their fingerprints have cleared, they may not hold project meetings or work with 4-H members.

 Educational Quiz

All 4-H volunteer leaders are encouraged to complete this ongoing educational section no later than December 1, 2009. Below are the four areas that you must review. You do not need to complete all sections at the same time. You can come back to this page at any time.

Read over the information on each page. (You might find it helpful to print out the page.) After you are familiar with the information, take the quiz/survey at the bottom of each page. (It's not necessary to take the quiz right away. You can take some time to read over everything, and come back to the quiz later.) Each quiz is very short and you can refer to your printed sheet if you need help with the answers.
(You need Adobe Acrobat Reader to view some of the information in the sections below. Download it for free from the Adobe website.)

1) Safety Awareness

2) Code of Conduct

3) Job Descriptions

4) 4-H Volunteer Forms

5) Child Safety

After you complete each section, a response will be automatically sent to the UCCE office, indicating that you have met the ongoing education requirement.
