Use the Solano County 4-H Incident Report to report all non-medical incidents that pose health & safety concerns or potential violations of California 4-H Member or Leader Codes of Conduct.
Use the Hartford Medical Insurance Claim Form to report all injuries and medical incidents (both youth & adult). Bring a blank form with you on every 4-H outing or activity. If an injury occurs, immediately complete section II of the claim form and call the UCCE office to report the incident.
Use the UC Incident Report Form to report incidents at 4-H activities that result in injury to anyone other that 4-H members or certified volunteer leaders.
The Project Leader Agreement was devised several years ago to help project leaders get organized, and to encourage them to think about what they plan to accomplish in the project.
The responsibilities of project leaders are listed in this document.
An Adult Medical Release Form must be completed annually by all leaders.
A Youth Medical Release Form must be completed annually by all youth (and signed by a parent/guardian).
The 4-H Club Program Planning Guide has been designed to assist you in completing the requirements for maintaining your 4-H club's charter and permission to use the 4-H name and emblem.

Community Club Leader Resources
Community Club Leaders should complete a Club Report to Leaders Council and return it to your Area Representative before each Leaders Council Meeting.
Program Fees are used to cover accident/illness insurance for participants, administrative curriculum, enrollment forms, operating costs for statewide committees, projram costs for statewide advisors, and program salary support.
Below are examples of enrollment instructions that some club leaders have found to be very helpful.
The 4-H Membership Attendance Policy describes the attendance requirements. Also included are sample letters to be sent to members who have missed meetings.
Use these forms to update your club's directory pages, club profile, club changes, club agreement, or annual inventory reports.
Outreach Documentation describes the efforts your club has made to recruit new members from your community.
Find out about available resources in the 4-H Audio Visual Resource Directory. These slides, videos, DVDs, and CDs are available for check-out from the Cooperative Extension office. A deposit is required but will be refunded upon the timely return of the items.
